MR Imaging of Brain was performed using T1W and Fast spin echo T2W high resolution Axial images were obtained on a dedicated phased array head coil using 3 tesla gradient system and correlated with T2W Saggital, Coronal and T2 Flair Axial images.

History - Patient came with the history of headache & Seizures.

Findings

There is evidence of a large ill defined area of signal alteration in the right temporal region with marked thickening of the overlying cortex showing loss of gray- white differentiation with involvement of the right gangliocapsular region infiltrating in to the corpus callosum with spread in to the contralateral hemisphere appearing hypointense on T1W and hyperintense on T2W and STIR images with scattered areas of hemorrhage and necrosis with marked surrounding vasogenic edema appearing hyperintense on T2W and FLAIR images with effacement of the sulcal spaces with subfalcine herniation to the left side and mass effect over the ventricular system which is effaced.

Opinion

Imaging features suggest possibility of A Glial Neoplastic Etiology (Gliomatosis Cerebri / Gliosarcoma / Anaplastic Astrocytoma) Vs A Gangliocytoma appears likely
